It also provides an opportunity for you to nominate one or more trusted friends or family members as your substitute decision-makers. Having this document in place can offer peace of mind, knowing that your preferences will be understood, should the need arise for someone else to make decisions on your behalf.

But where do you start? The City of Victor Harbor, District Council of Yankalilla, and Alexandrina Council, with the support of grant funding from Office for Ageing Well and Health Services Programs, SA Health, have established the Fleurieu Advance Care Directive Program.

This free program supports community members to plan for the future with confidence and clarity. Trained volunteer peer support officers are available for one-on-one appointments, providing information on navigating the form and supporting community members through the process of creating their own Advance Care Directive.

Booking an appointment with a trained peer support officer is easy, with sessions available at council offices in Victor Harbor, Goolwa, and Strathalbyn, as well as the Yankalilla Library.

It is important to remember that while this service offers general information, if you need specific advice regarding the legal or medical aspects of the Advance Care Directive form you will need to seek support from professional services.
