An Advance Care Directive (ACD) empowers you to make clear legal arrangements for your future healthcare, end of life, preferred living arrangements and other personal matters. However, many older people do not have a current or fully completed ACD containing their wishes for their future health and lifestyle.

The Southern Councils of Onkaparinga, Marion, Mitcham and Holdfast Bay, supported by Office for Ageing Well and Health Services Programs, SA Health.  We are looking for change-makers, aged 50 years and over, to join us to help codesign a new peer-led workshop and accompanying resources that will assist community members to understand and complete their own Advance Care Directive.

You can contribute to this valuable project now, through a community survey, and share your views about ACDs, whether you have a completed document, are working on a copy, or have never considered one before. This data will help the project team understand the barriers and design solutions to support more people to express their wishes for the future.

Co-design workshops will be held in 2022 to ensure the resources developed from the project meet the needs of the community.
